Job Summary

This is a rewarding PRN/on-call position that requires providing high-quality, compassionate end-of-life care to patients in their home or community setting after regular business hours, weekends, and holidays. The ideal candidate will be a registered nurse with at least 5 years of experience in hospice or acute care settings, preferably certified in hospice and palliative care.

The successful candidate will possess excellent communication and interpersonal skills, as well as the ability to work independently and make sound clinical judgments in high-pressure situations.

Responsibilities
  • Assess patient needs and provide support via phone or visit, as needed, in accordance with standards of practice and evidence-based knowledge for hospice programs.
  • Implement care plans to achieve outcomes of comfortable and safe dying, self-determined life closure, and effective grieving for patients and families.
  • Document all clinical nursing visits, medication additions and changes, care plans, and additional documentation within 24 hours of provided service.
  • Provide education to household members, facility staff, and volunteers on nursing or personal care needs.
Requirements
  • Certified Hospice and Palliative Care Nurse (CHPN) certification is preferred but not required.
  • A valid driver's license and daily access to a reliable vehicle are mandatory.
  • Ability to work independently and prioritize tasks effectively.
  • Excellent written and verbal communication skills.
  • Current CPR certification is required.
