We are seeking a skilled Case Management Specialist to join our team at Quality Angels Human Service. The successful candidate will have a strong background in case management, with the ability to develop and implement effective individualized plans that meet the unique needs of our clients.

Responsibilities:

  • Develop and implement effective individualized plans that meet the unique needs of our clients.
  • Conduct regular assessments and updates, including initial and annual assessments.
  • Communicate regularly with clients' Support Coordinators and families regarding progress, changes in care, and any concerns.
  • Maintain accurate and up-to-date client records, including progress notes and incident reports.

Requirements:

  • Bachelor's degree in Human Services, Social Work, Psychology, or a related field.
  • Minimum of 2 years of experience working with individuals with intellectual and developmental disabilities (IDD).
  • Knowledge of relevant regulations and standards, including 6400.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple projects simultaneously.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and colleagues.

Estimated Salary: $55,000 - $70,000 per year, depending on experience.
