About the Role

**Job Summary:**

We are seeking a highly skilled Psychiatric Rehabilitation Services Coordinator to join our team at Transitional Care Management. As a key member of our interdisciplinary team, you will provide group and individual therapy, psychiatric rehab, and case management services to adults with a history of psychiatric hospitalizations.

Key Responsibilities:

  • Develop and implement therapeutic services and interventions to best serve the needs of our patients.
  • Provide group and individual therapy, including psychiatric rehab and case management services.
  • Collaborate with our interdisciplinary team to develop and implement treatment plans.
  • Conduct assessments and evaluations to identify patient needs and develop effective treatment plans.
  • Provide education and support to patients and their families on mental health issues and treatment options.

Requirements:

  • Bachelor's Degree in a human services field, such as sociology, special education, rehabilitation counseling, or psychology.
  • Minimum of 1 year of supervised experience in mental health or human services setting.
  • Experience with Point Click Care (PCC) is ideal, but not required.
  • Prior experience as a PRSC or mental health professional in a licensed nursing home, intermediate care facility, or specialized mental health rehab facility (SMHRF) is preferred.
