Job Title: Travel Nurse RN

Cynet Health is seeking a skilled Travel Nurse RN to join our team. As a Case Manager, you will provide individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or terminal illnesses.

Responsibilities:
  • Perform admission, concurrent, and retrospective reviews on all patients.
  • Ensure accurate status and level of care determination upon admission.
  • Obtain and document pre-certifications/authorizations for all admissions, surgeries, and procedures.
  • Follow Medicare guidelines regarding utilization review.
  • Submit telephonic and/or fax notification/clinical information.
  • Follow up with insurer for authorizations.
  • Identify potentially high-risk patients and refer to Social Work.
  • Provide effective clinical documentation and conduct Initial Assessments to develop a plan of care.
  • Ensure treatment plans are implemented, re-evaluated, and updated daily.
  • Facilitate effective communication between members of the treatment team.
  • Coordinate meetings with families and treatment teams.
  • Facilitate arrangement of post-acute care services.
  • Act as an advocate for patients/families.
  • Follow Medicare guidelines regarding patient choice, admit/discharge notifications, and appeals.
  • Maintain accountability for facilitating clinical patient progression through a defined plan of care to achieve optimal outcomes.
Requirements:
  • Graduate of an accredited school of nursing.
  • License in good standing.
  • Minimum of one year of current Case Manager experience in a facility.
  • Evidence of current AHA BLS credential mandatory.
