- Collaborate with the multidisciplinary team to achieve desired patient care outcomes, communicating effectively with patients, families, and healthcare professionals.
- Provide education and support to patients and families, promoting health and wellness through evidence-based practices.
- Participate in quality improvement initiatives, contributing to the development and implementation of policies and procedures that enhance patient care and safety.
- Document patient information accurately and timely, maintaining confidentiality and adhering to HIPAA regulations.
- Current licensure as a Licensed Practical Nurse in the state of practice.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Ability to assess and prioritize patient needs, making sound judgments and decisions in a fast-paced environment.
- Basic computer skills and proficiency in electronic medical records.
