Overview:
The Practical Nursing Specialist is a licensed professional dedicated to providing guidance to clients and caregivers on managing health conditions as part of our Health Corner initiatives. The primary objective is to ensure effective utilization of Health Corners.
In the role of Health Advisor, the Practical Nursing Specialist will interact with patients and caregivers to address health-related inquiries and supply educational resources and information. If deemed necessary, referrals to other clinical providers may be made. It is important to note that the Practical Nursing Specialist will not engage in diagnosing or prescribing treatments based on patient conditions, adhering strictly to the scope of practice defined by professional licensing bodies and company policies.Key Responsibilities:
- Supervise Health Corner operations and provide both in-store and online diagnostic tools and solutions, assisting customers with proper usage.
- Administer vaccines (both intramuscular and subcutaneous) following physician protocols.
- Collect blood samples as requested by a physician or under established protocols.
- Conduct gap closure activities as outlined and escalate issues as necessary.
- Effectively manage a recurring patient caseload each month.
- Coordinate care for a defined patient population to address both clinical and non-clinical gaps.
- Assess customer needs and advocate on behalf of patients to connect them with resources and services related to their health conditions.
- Identify and coordinate community resources for patients.
- Conduct telephonic outreach to patients.
- Educate patients on the proper setup and use of diagnostic devices (e.g., blood pressure monitors, glucose meters, pulse oximeters).
- Communicate effectively with agency staff, liaisons, other healthcare facility personnel, patients, and their families/caregivers.
- Manage time efficiently to provide accurate and appropriate patient care.
- Maintain confidentiality of patient and proprietary information.
- Uphold a professional image at all times.
- Participate in required training sessions, in-services, and weekly meetings.
- Build and sustain a collaborative relationship with pharmacy and front-end teams to enhance patient engagement and experience.
- Assist patients in-store by providing education on over-the-counter and first aid products, guiding them to the pharmacy for specific inquiries.
- Maintain the Health Corner space in accordance with brand standards and keep organized records as per compliance policies.
Basic Qualifications:
- Active LPN/LVN license as per State Board of Professional Regulations (must remain current with licensure, certifications, and continuing education).
- A minimum of 2 years of experience as an LPN/LVN providing nursing care.
- Self-motivated with excellent time management skills, capable of working independently as well as under the supervision of a registered nurse or another clinician.
- Willingness to obtain necessary certifications prior to employment.
- Experience with diverse patient populations, including children and the elderly.
- Flexibility and ability to work effectively within a multidisciplinary team.
- Current CPR/BLS certification and up-to-date on bloodborne pathogen training.
- Effective communication skills (both verbal and written) with team members, patients, caregivers, and healthcare providers.
- Willingness to work a flexible schedule, including evenings and weekends.
- Comfortable with digital technologies, including website forms, mobile applications, and connected healthcare devices.
- Ability to perform under stressful conditions.
- Reliable transportation to the job site.
Preferred Qualifications:
- Bilingual in English and Spanish (may be required for certain locations).
- Two years of experience in a retail or clinic environment (familiarity with local healthcare systems and health plans is a plus).
- Experience in health coaching, particularly in areas such as diabetes management, colorectal screening, blood pressure monitoring, and health risk assessments.
- Proven ability to assist customers in resolving complex issues.
- Advanced computer skills, including document scanning and uploading, with typing proficiency of 40-50 WPM.
- Prior experience with electronic health records (EHR) or electronic medical records (EMR).
