Job Summary

Key Responsibilities
  • Provide social work services under the direction of healthcare professionals to meet the physical, mental, and emotional needs of patients.
  • Conduct initial social work assessments for adult and geriatric patients, identifying areas of need and developing personalized plans for care.
  • Participate in treatment planning, discharge planning, and referrals to ensure seamless care coordination.
  • Provide crisis intervention, supportive counseling, and problem-solving interventions as needed.
  • Collaborate with multidisciplinary treatment teams to assess treatment needs, develop strategies, and set goals for patient care.
  • Document patient information accurately and maintain confidentiality of all records.
  • Develop and provide individualized and group counseling, guidance, and problem-solving assistance to patients and families.
  • Assist in scheduling patients for appointments and maintaining accurate records of patient information.
  • Provide health education and resources to patients and families to promote informed decision-making.
  • Participate in performance improvement initiatives to enhance patient care and services.
Requirements
  • Master's degree in Social Work from a college or university accredited by the Council on Social Work Education.
  • Psychotherapy certification.
  • Current licensure in the state where the MSW/LCSW practices.
  • Current Cardiopulmonary Resuscitation (CPR)/Basic Life Support (BLS) certification.
  • Knowledge of professional social service principles, procedures, and techniques.
  • Ability to assess patient condition, recognize adverse signs and symptoms, and react swiftly in emergency situations.
  • Effective verbal and written communication skills, including proper telephone etiquette.
  • Proficient usage of medical terminology and Microsoft Office software.
  • Working knowledge of basic computer maneuverability.
  • Minimum one year of experience working with mental health/psychiatric patients, preferably in a clinical setting.
