Job Summary

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Tyler Family Circle of Care. As a key member of our healthcare team, you will provide exceptional patient care and support to our family medicine patients.

Key Responsibilities
  • Provide advanced nursing care and physician support to family medicine patients in a clinic setting.
  • Work within established protocols and multidisciplinary plans of care, and clinical specific standards under the direction of a Physician.
  • Participate in a Patient Centered Medical Home (PCMH) team and be familiar with the philosophy, goals, and objectives of PCMH.
  • Perform procedures such as injections and immunizations when needed.
  • Assess, interpret, and provide age-specific care to patients.
  • Document pertinent subjective and objective findings of health conditions and formulate management plans.
  • Communicate plans to patients and involve family in decision-making when appropriate.
Requirements
  • Master's degree or equivalent in advanced nursing required.
  • At least one year of experience as an RN in Acute or Ambulatory Health Care Setting preferred.
  • Current RN license for Texas and Family Medicine NP license/certification in Texas required.
  • Current BCLS Healthcare Card and prescribing authority/DEA licensure in good standing required.
