Job SummaryWe are seeking a highly motivated and experienced Registered Nurse to join our team as a Hospice Nurse in Flint, MI. As a key member of our care team, you will be responsible for providing end-of-life care and support to patients and their families, working collaboratively with our interdisciplinary team to develop and implement individualized care plans.
Key Responsibilities- Provide constant and ongoing care for elderly and terminally ill patients, evaluating patient needs and creating care plans to ensure optimal outcomes.
- Work in a collaborative, team-driven environment, communicating effectively with other care team members to ensure seamless care delivery.
- Assess and reassess patient needs, providing dietary counseling and initiating nursing procedures as needed.
- Teach, supervise, and counsel patients and their families on providing care for the patient, ensuring continuity of care and promoting independence.
- Develop and re-evaluate patient/family care plans in conjunction with the IDG, ensuring needs are met and care is maintained.
- Perform specific nursing procedures as needed, following doctor's orders and adhering to agency policies and procedures.
- Attend team conferences and participate in on-call system, providing on-call coverage when unavailable for assigned duties.
- Licensed or registered in the state in which currently practices.
- Must pass a criminal background check and MVR check.
- Completed health statement acknowledging ability to perform the duties of the position.
- Valid state driver's license and proof of current automobile insurance.
- Current CPR card and TB testing per agency policy.
