Key Responsibilities:
- Conduct thorough assessments and evaluations of patients to ascertain their physical capabilities, limitations, medical histories, and personal aspirations.
- Analyze patients' abilities to perform specific functional tasks.
- Deliver direct patient care as authorized by a physician.
- Formulate and revise treatment plans, including testing and measuring patients' strength and flexibility.
- Collaborate with physicians, patients, families, nurses, therapists, social workers, and other members of the healthcare team.
- Develop tailored treatment plans with clear goals and tasks to assist patients in achieving their functional objectives.
- Demonstrate exercises that empower patients to better manage daily activities and alleviate discomfort.
- Assess patients' living or working environments to recommend enhancements based on their unique needs.
- Educate families on how to support and accommodate their loved ones effectively.
- Monitor patients' progress towards goals and prepare comprehensive reports for healthcare providers and insurance entities.
- Ensure timely completion of all clinical documentation using the company's electronic medical record system.
- Participate in meetings and case discussions to guarantee coordinated and holistic care plans for patients.
- Contribute to revenue generation by accurately recording billing information for services rendered.
