As a key member of our healthcare team, you will play a vital role in facilitating care along a continuum, ensuring that patients receive the most appropriate level of care based on medical necessity.
Key Responsibilities- Coordinate care across the continuum, ensuring seamless transitions and efficient throughput.
- Assess patients for transition needs, promoting timely discharge and preventing avoidable readmissions.
- Integrate national standards for care management, including utilization management, transition management, and care coordination.
- Ensure compliance with state and federal regulatory requirements, as well as Tenet policy and accreditation standards.
- Provide education to physicians, patients, families, and caregivers on care management best practices.
- **Education:** Graduate of an accredited School of Nursing.
- **Experience:** 2 years of acute hospital patient care experience, with utilization/case management experience preferred.
- **Certifications/Licenses:** Possession of current Texas State license for Registered Nurse, with Accredited Case Management (ACM) certification preferred.
- **Courses/Completions:** Active healthcare provider Basic Life Support certification (ARC or AHA), with AHA Healthcare Provider BLS certification required within 60 days of hire.
