We are seeking an experienced Patient Care Director to lead our hospice operations team. The successful candidate will be responsible for overseeing the day-to-day clinical and administrative functions of our hospice sites, ensuring high-quality patient care and excellent staff performance.

Key Responsibilities:
  • Supervise and manage a team of healthcare professionals, including RNs, LPNs, and CNAs
  • Develop and implement clinical policies and procedures to ensure compliance with regulatory requirements
  • Conduct regular site visits to monitor patient care and staff performance
  • Manage budgets, resources, and personnel to optimize operational efficiency
  • Collaborate with multidisciplinary teams to coordinate patient care and achieve quality outcomes
Requirements:
  • Degree in nursing (ASN or BSN) from an accredited school
  • At least 1 year of experience as a registered nurse
  • 3 years of experience in direct patient care, preferably in hospice or home health
  • Proven leadership skills and ability to manage a team
  • Excellent communication, problem-solving, and analytical skills
