Key Responsibilities
  • Conduct comprehensive assessments of patients' needs and develop personalized treatment plans to improve mobility, reduce pain, and restore function.
  • Provide physical therapy services to patients in their own homes, adapting treatment plans to suit home environments while ensuring high-quality care and patient safety.
  • Collaborate with physicians and other healthcare professionals to develop and implement interdisciplinary client care plans.
  • Teach, supervise, and counsel patients and their families on physical therapy programs, including the use of adaptive devices.
  • Record and report evaluation data, treatments, and patients' responses to therapy intervention.
  • Participate in case conferences, staff meetings, and in-service programs to stay up-to-date on best practices and agency policies.
Requirements
  • Graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association.
  • Licensed or registered by the State.
  • At least one year of institutional experience.
  • Home care experience and at least one year of supervisory experience (2 years preferred).
  • One year of experience in an acute/sub-acute setting.
