Job Title: Registered Nurse

Job Summary:

The Registered Nurse is a key member of our team, responsible for planning, organizing, and directing home care services. With a focus on community health education and experience, this role requires a professional RN to build from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Responsibilities:

  • Plan and direct home care services to meet the needs of individuals and families
  • Build relationships with community resources to support patient care
  • Collaborate with healthcare teams to ensure seamless care transitions
  • Provide education and support to patients and families on health management and self-care
  • Conduct assessments and develop care plans to meet patient needs

Requirements:

  • Current and unencumbered RN license in the state of employment
  • Bachelor's degree in Nursing from an accredited program (preferred)
  • One to two years of recent acute care experience in an institutional setting
  • One year of home health care experience (preferred)
  • Management experience not required, but direct supervisory experience of LVNs and NAs/As (preferred)
  • Possess and maintain valid CPR certification
