Responsibilities- Provide accurate and comprehensive assessments, diagnoses, and treatment plans for patients with various medical conditions.
- Conduct thorough history and physical examinations, and document all patient care within an electronic health record.
- Counsel and educate patients on health maintenance, disease prevention, and treatment options.
- Work effectively with the clinical care team to ensure seamless care coordination and communication.
- Participate in clinical care team huddles and contribute to the development of care plans.
- Collaborate with specialists and other healthcare professionals as needed to ensure comprehensive patient care.
- Manage and maintain accurate and up-to-date patient records, including lab results and test data.
- Develop and implement effective treatment plans, taking into account patient preferences, values, and cultural backgrounds.
- Stay up-to-date with the latest medical research, guidelines, and best practices in primary care.
- Master's degree in Family Nurse Practitioner with current National Board Certification and State of Employment license to practice in the Advanced Practice role.
- A minimum of 1 year experience as a Nurse Practitioner, with experience in Family medicine or primary care practice preferred.
- Active Advance Practice Registered Nurse, Nurse Practitioner license in good standing.
- Current National Board Certification in Family Practice.
