Rehabilitation Liaison

Job Summary:

The Rehabilitation Nurse Liaison plays a vital role in maintaining communication with designated healthcare facilities, performing on-site pre-admission screenings, and providing patient care services to older adolescent, young adult, adult, and geriatric populations.

Key Responsibilities:

• Maintain effective communication with healthcare facilities to ensure seamless patient care transitions.
• Perform on-site pre-admission screenings to assess patient needs and develop individualized care plans.
• Provide patient care services to older adolescent, young adult, adult, and geriatric populations, including those with complex medical needs.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Participate in marketing efforts to promote rehabilitation services and educate healthcare professionals about our programs.

Requirements:

• CRRN certification preferred, obtained within 3 years of working on the Rehab unit.
• Ability to work PRN or Per Diem as needed.
