Artesia General Hospital seeks a skilled Case Manager to oversee the functions of case management and discharge planning. The ideal candidate will possess strong communication skills and the ability to work effectively with patients, families, and healthcare providers.
Key Responsibilities:
Establish and maintain open communication with patients, families, and healthcare providers
Determine patient medical eligibility and Medicare/Managed Care eligibility
Identify strategies to reduce in-patient length of stay and resource consumption
Provide patients and families with information about home health care, skilled nursing facilities, and rehabilitation facilities
Requirements:
Graduate from an accredited school of nursing (BSN preferred)
Five years of position-related experience in a healthcare-related field
Current New Mexico Nurse License and certification in case management (preferred)
Work Environment:
The Case Manager will work in a fast-paced healthcare environment, interacting with patients, families, and healthcare providers on a daily basis. The ideal candidate will be able to work effectively in a team environment and maintain confidentiality and security of patient information.
