About the Role

We are seeking a skilled Occupational Therapist to join our Home Health team at EvergreenHealth. As an Occupational Therapist, you will play a vital role in helping patients achieve their maximum functional status and independence in their own homes.

Key Responsibilities
  • Assess, plan, implement, and evaluate skilled care to patients requiring occupational therapy services in the home.
  • Provide instruction and education to patients and/or caregivers to promote maximal functional status.
  • Regularly evaluate and re-evaluate patient status, and initiate, develop, implement, and revise the patient's plan of care as needed.
  • Communicate with other disciplines involved in the case as warranted, and report signs/symptoms signaling a change in patient condition to the patient's physician and case manager as needed.
Requirements
  • Current OT licensure to practice Occupational Therapy in Washington State.
  • Recent applicable clinical experience in a medical/surgical unit, rehab, or home health setting.
  • Current Washington State Driver's license and proof of insurability.
  • Current Healthcare Provider BLS certification by date of hire.
  • Reliable transportation.
Preferred Qualifications
  • Bachelor's, Master's, or Doctorate in Occupational Therapy.
