Job Title: Clinical Coordinator RN

DaVita is seeking a skilled Clinical Coordinator RN to join our team. As a Clinical Coordinator, you will play a vital role in delivering high-quality patient care and services in our outpatient dialysis center.

Responsibilities:
  • Coordinate ESRD patient care and services, including trend lab values, tracking metrics on vital signs, weight, and outcome management.
  • Ensure the safety, comfort, and well-being of patients by delivering exceptional care and services.
  • Develop and propose solutions to atypical problems based on experience and analysis.
  • Guide, coach, and train other teammates to enhance overall performance and clinical outcomes.
  • Lead CQI program and outcome management, and drive improvement initiatives.
  • Participate in Facility Health Meeting (FHM) and contribute to the development of quality improvement initiatives.
Requirements:
  • Current Registered Nurse (RN) license in the state of practice.
  • At least 18 months of registered nursing experience.
  • Dialysis Charge RN readiness approval by Clinical Nurse Manager or RN-licensed FA/GFA required.
  • Current CPR certification required.
  • Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN) preferred.
  • Intensive Care Unit (ICU), Critical Care Unit (CCU), Emergency Room (ER), or Medical Surgical (Med Surg) experience is preferred.
  • Associates Degree in Nursing (A.D.N) from accredited school of nursing required; Bachelor of Science in Nursing (B.S.N) preferred; three-year diploma from accredited diploma program may be substituted for nursing degree.
  • Basic computer skills and proficiency in MS Word and Outlook required.
