Job Summary

We are seeking a skilled Nurse Practitioner to join our team on a PRN/Part Time basis. As a Nurse Practitioner with Matrix Medical Network, you will conduct comprehensive health assessments, develop care plans, and provide education to patients and their families.

Responsibilities
  • Conduct adult and geriatric assessments, medical history, diagnosis, and treatment
  • Develop and implement care plans to improve patient outcomes
  • Provide education to patients and their families on health management and disease prevention
  • Collaborate with primary care physicians, case managers, and other healthcare professionals to ensure seamless care coordination
  • Work in a variety of settings, including home visits, skilled facilities, and telehealth visits
Requirements
  • Masters degree in nursing or related field
  • Current RN and NP licensure in state of practice
  • Board certification in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
