Job Summary

This RN Patient Navigator role supports Interventional Radiology to assess, address, and prevent barriers to care throughout the complex healthcare continuum. As a key member of the multidisciplinary team, you will collaborate with patients, families, and caregivers to identify and overcome obstacles that may hinder quality medical and psychosocial patient care.

Key Responsibilities
  • Assess patient plans of care and develop, implement, monitor, and document the utilization of resources and progress of the patient through their care.
  • Evaluate medical necessity and appropriateness of care, optimizing patient outcomes, and serving as a liaison for referring physicians.
  • Assist patients and families through diagnostic services, treatment, and care, coordinating disease care using an interdisciplinary holistic approach.
  • Ensure patients and families understand diagnostic processes, care strategies, and recommended actions, and provide appropriate information in coordination with physicians and healthcare providers.
  • Evaluate patient functional abilities and limitations, determine if intervention is needed, and establish treatment goals that are functional, measurable, patient-related, and reflect key limiting factors.
  • Support patients and families during difficult decision-making periods, assist in coordination of end-of-life care, and provide emotional support.
  • Support process improvement activities for populations of patients to achieve optimal clinical, financial, operational, and satisfaction outcomes.
Requirements

This role requires a Bachelor's degree in a related field and a strong understanding of healthcare systems, patient care coordination, and communication skills. The ideal candidate will be able to work independently, with freedom to determine how to best accomplish functions within established procedures, and will have a collaborative relationship with physicians, payers, and other members of the healthcare team.
