Job Title: Registered Nurse

Community Healthcore is seeking a skilled Registered Nurse to join our team. As a Registered Nurse, you will be responsible for providing high-quality medical care to our clients in accordance with nursing licensure standards and established policies and procedures.

Key Responsibilities:
  • Coordinate the delivery of Medicaid waiver and non-waiver services in compliance with the IDD provider principles for evidentiary certification.
  • Assist the QIDP/Case Manager in developing individualized services for each consumer based on their needs, preferences, strengths, and assessments.
  • Advocate for and ensure consumer/family/legal guardian satisfaction in services.
  • Function as a team member with the QIDP/Case Manager, Home Manager, Program Manager, Program Director, Director of Nursing, and Direct Care staff to deliver services according to the plan of care.
  • Monitor the delivery of medical services and provide RN delegation for non-medical staff.
Requirements:
  • Graduate from an accredited school of nursing.
  • Licensed to practice as a Registered Nurse in the State of Texas.
  • Current CPR certification.
  • Must have a driving record insurable by the CHC's insurance administrator.
Preferred Qualifications:
  • Demonstrates knowledge of standards of nursing practice for Registered Nurses.
  • Knowledge of the nursing peer review process.
  • Working knowledge of licensing standards for services and programs of the center.
Physical Requirements:

The position requires various activities involving visual, auditory, motor skills to communicate, attend meetings/trainings, carrying charts, and assisting with individuals (up to 50 lbs.) during an 8-hour day. The nurse must also have fine-motor skills for legible and accurate writing of reports, charting, scheduling, and daily correspondence, as well as endurance to work an average 8-hour day - up to 12 hours may be required on isolated occasions.
