Physical Therapist Responsibilities

  • Conduct comprehensive assessments and develop personalized plans of care for patients
  • Regularly evaluate patient progress and adjust treatment plans as needed
  • Collaborate with physicians and other healthcare professionals to ensure seamless care coordination
  • Participate in interdisciplinary team meetings and care conferences to discuss patient needs and develop comprehensive care plans

Physical Therapist Requirements

  • Current licensure as a physical therapist in the state of Michigan
  • At least one year of experience as a physical therapist in the last five years
  • Active CPR certification
  • Previous home health experience preferred, but not required
  • Reliable transportation with a valid state driver's license and car insurance
