Position Overview:

This role involves directing and delivering nutritional care to patients through comprehensive assessment, strategic planning, execution, and ongoing evaluation; it also includes providing nutrition education to both patients and healthcare staff.

Key Responsibilities:

  • Conducts personalized patient evaluations and follow-ups. (30%)
  • Creates, documents, and oversees the nutritional care plan tailored to the specific needs of the patient population in accordance with professional standards. (30%)
  • Delivers educational sessions and/or programs aimed at enhancing the knowledge of patients and staff. (20%)
  • Completes documentation and reports in line with established procedures and practices. (10%)
  • Develops, implements, and assesses tools necessary for improving the nutritional care provided to patients, staff, and the community. (10%)
  • Performs additional duties as assigned or required.

Qualifications:

Minimum Requirements:

  • Bachelor's Degree in Dietetics or Nutrition
  • Registered Dietitian Nutritionist (RDN) OR Registered Dietitian (RD) OR eligible and must obtain either RDN or RD within six months of employment.
  • Licensed Dietitian/Nutritionist (LDN) OR eligible and must obtain within nine months of employment.

Preferred Qualifications:

  • Master's Degree
  • Experience in specialized practice areas.
