Job Title: LPN/LVN Care Specialist

Join CenterWell Senior Primary Care as an LPN/LVN Care Specialist and be part of a team dedicated to providing proactive, preventive care to seniors. As a key member of our care team, you will play a vital role in delivering high-quality patient care and promoting whole-person health.

Responsibilities:
  • Provide professional nursing care to patients within an assigned unit, clinic, or region.
  • Assess patient health problems and needs, take vital signs, administer medication, and maintain medical records.
  • Advise patients on health maintenance and disease prevention or provide case management.
  • Collaborate closely with physicians and nurses to ensure top-level patient care.
  • Perform in-clinic routine tests, screening, and point-of-care testing, such as blood draws, EKGs, and vaccinations.
  • Schedule and assist with external routine screening and diagnostic tests, such as mammograms and echocardiograms.
  • Medication adherence and refilling medications.
  • Prepare and administer medications, including intramuscular, intradermal, and subcutaneous injections.
  • Counsel patients on medication and special diets.
  • Wound care and changing dressings.
  • Sterilize and clean equipment and ensure maintenance of examination rooms.
Requirements:
  • Current licensure as an LPN or LVN.
  • Phlebotomy experience.
  • Reliable transportation.
  • Excellent customer service.
  • Strong clinical background.
  • The ability to adapt and work in multiple clinical settings.
  • Strong sense of engagement to be a team player.
  • Experience in a fast-paced/high-volume environment.
Preferred Qualifications:
  • 2 years of clinical experience.
  • Hands-on professional.
  • Phlebotomy experience.
  • Experience with Electronic Medical Records.
  • Experience with HEDIS.
  • Knowledge of primary care medicine requirements (state-specific).
