We are seeking a highly skilled Clinical Pharmacist to join our team in the Critical Care/Emergency Medicine department. As a Clinical Pharmacist, you will play a critical role in providing comprehensive medication management to optimize medication use for critical care/emergency medicine patients.
Responsibilities- Coordinate and provide pharmaceutical care for adult critical care patients and emergency medicine patients.
- Work collaboratively with the healthcare team to ensure seamless patient care.
- Develop and implement medication therapy plans to optimize patient outcomes.
- Provide education and training to healthcare staff on medication management and safety.
- Participate in quality improvement initiatives to enhance patient care and safety.
- Bachelor's Degree in Pharmacy from an accredited school of Pharmacy.
- State licensure as a Pharmacist and any relevant life support certification as determined at position level.
- Completion of a PGY2 critical care residency or PGY1 pharmacy practice residency with critical care experience are preferred.
