About the Role

We are seeking an experienced Comprehensive Case Manager and Housing Specialist to join our Enhanced Care Management Team. The ideal candidate will possess excellent problem-solving skills, knowledge of community resources, and a passion for making a difference in the lives of individuals.

Job Summary

This full-time role is office-based, with up to 30-80% travel required for face-to-face visits with clients within our service region. You will work closely with a care management team to provide comprehensive case management services, assist participants with finding community resources, engage in advocacy efforts, and respond timely to questions and inquiries.

Key Responsibilities
  • Work with the care management team to provide comprehensive case management services
  • Assist participants with applications for services, such as health benefits and insurance coverage, IHSS, CalFresh, low-income housing, and Waiver and managed care programs
  • Engage in advocacy efforts to ensure participants receive timely medical services
  • Respond timely to questions and inquiries from the care management team, participants, and clinical and administrative staff representing other agencies
  • Provide education to participants on community support programs
  • Document consumer interaction and maintain timely reporting and documentation requirements
Requirements
  • Program knowledge and experience with HUD and housing programs
  • Current CPR certification
  • Valid California Driver's License
  • Eligible to be insured under our liability policy with a clean DMV report
  • Your own car, and proof of current auto insurance
  • Covid vaccinated or have an exemption
  • Pre-employment background check and fingerprinting compliance
Preferred Skills and Qualifications
  • Experience providing care management services and knowledge of low-income housing resources
  • Ability to communicate effectively in both writing and verbally with colleagues, supervisors, and participants
  • Ability to work in the community for face-to-face visits with clients and in the office environment
  • Knowledge of basic problems and needs of persons diagnosed with a physical disability or emotional disturbance
  • Knowledge of community and supportive services
  • Skilled in assessing and prioritizing multiple tasks, projects, and demands
Compensation and Benefits

The pay range for this position is between $22-$25 per hour, with exact compensation varying based on skills, experience, and location. You will also be reimbursed for mileage traveled from the office to participant homes and receive a monthly phone stipend. Additional benefits include health insurance, dental insurance, vision insurance, life insurance, and a Flexible Spending Account (FSA) after 60 days of employment.
