Job OverviewAt A Special Touch Home Health Care Ll, we are seeking a skilled and compassionate Licensed Practical Nurse to join our team on a part-time basis. The ideal candidate will possess excellent nursing skills, a strong work ethic, and the ability to work independently with minimal supervision.

Key Responsibilities:
Admit referrals for Personal/Respite care
Create and update plan of care as necessary
Document and communicate actions to maintain continuity of care
Comply with procedures, regulations, rules in maintaining medical records
Perform assessments on clients to assess their conditions and home environments
Teach clients and their families about health care in the home
Data entry submission of documents electronically as required for various authorizations

Requirements:
Active Applicable Virginia License
American Heart Association issued BLS and ACLS
At least 2 years of experience as an LPN and 1 year in the home health industry
Dependable transportation
