We are seeking a compassionate and skilled Certified Nursing Assistant to join our team at Hospice of the Midwest. As a member of our team, you will play a vital role in providing personalized care and support to patients and their families.
Responsibilities- Provide personal care services, including bathing, dressing, and grooming, as needed.
- Assist with daily living activities, such as feeding, toileting, and transferring patients.
- Monitor and report changes in patient condition to the Case Manager.
- Participate in quality assessment and performance improvement activities.
- Collaborate with the hospice team to ensure comprehensive care and support.
- Meet the training requirements of the State.
- Completed a minimum of seventy-five hours of classroom and supervised practical training.
- Minimum of six months experience in providing personal care.
- Ability to read and follow written instructions and document care given.
- Understands hospice philosophy and is comfortable providing specialized care to the terminally ill.
