Hospice Nurse Care Specialist

Concho Hearts Hospice is dedicated to providing compassionate end-of-life care to patients and their families in the comfort of their own homes. As a Hospice Nurse Care Specialist, you will play a vital role in our mission to deliver exceptional patient-centered care.
About the OpportunityWe are seeking a registered nurse with a passion for palliative care to join our team. In this role, you will administer nursing care to patients of all ages, coordinate care with our interdisciplinary team, and provide emotional support to loved ones and primary caregivers.
This is a PRN position, requiring a minimum of 2 days per week. With potential to become full-time, you will have the opportunity to grow your career and make a meaningful impact in our community.
Responsibilities- Develop Patient-Centered Care Plans: Prepare and adjust patient care plans as needed to ensure individualized care that meets the unique needs of each patient.
- Provide Education and Support: Educate loved ones and primary caregivers on patient care information, end-of-life education, and emotional support to empower them to navigate the care journey.
- Maintain Medical Records: Maintain accurate and up-to-date medical charts, patient files, and health records to ensure seamless communication between healthcare providers.
- Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
- Licensure: Current license as a registered nurse (RN) and/or by the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC).
- Experience: Two years of experience as a registered nurse in a clinical care setting is preferred, with hospice experience being an asset.
- Skills: Strong nursing skills, good interpersonal skills, and specialized skills in palliation and end-of-life care. Proof of current CPR and ability to read, write, and comprehend English.
- Transportation: Reliable transportation and valid auto liability insurance.
- Environmental and Working Conditions: Ability to work in patients' homes in various conditions, possible exposure to blood and bodily fluids and infectious diseases. Flexible schedule, local travel, and some exposure to unpleasant weather.
- Physical and Mental Effort: Prolonged standing and walking required, with ability to lift up to 50 lbs and move patients. Works under stressful conditions to meet deadlines and patient needs, and makes quick decisions and resource acquisition.
- Mileage Reimbursement: $0.50 per mile.
As a member of our team, you can expect competitive pay and benefits, including mileage reimbursement. With a salary range of $45,000 - $65,000 per year, depending on experience, you will be compensated fairly for your expertise and dedication to delivering exceptional patient care.
