As a Geriatric Medicine Specialist at Hackensack University Medical Center, you will be responsible for providing high-quality patient care and ensuring the quality of care within the Division of Geriatrics.
Key Responsibilities:- Provide Clinical Continuity of Care: Ensure seamless care for patients within the Division of Geriatrics.
- Perform History and Physical Examination Evaluations: Conduct thorough evaluations and review hospital records, labs, and verify information with patients' primary care physicians or specialists.
- Order and Provide Necessary Rests: Order and provide necessary tests and treatments based on patients' physical conditions.
- Follow Up on Test Results: Communicate with patients on the results of all test/examination reports.
- Communicate with Nursing Staff and Management: Collaborate with the Director of Nursing and Nursing Staff as well as Sub-Acute/Nursing home management as needed.
- Provide Call Coverage: Provide evening, night, and weekend call coverage as assigned.
- Transfer Patients: Transfer patients to and from HackensackUMC as per hospital policy.
- Communicate with Primary Care Physicians: Communicate relative information to patients' primary care physicians as needed.
- Communicate with Accountable Care Organization: Communicate with the accountable Care Organization as needed.
- Write Discharge Summaries: Write discharge summaries or transfer notes, prescribe medications, recommend labs, and notify primary physicians.
- Develop Quality Improvement Plans: Develop and monitor quality improvement plan components for geriatric internal medicine.
- Assist with Teaching and Education: Assist the Section Chief in designing and implementing teaching and education programs in nursing education.
- Provide Clinical Support: Assist the Section Chief in providing clinical support for geriatric/internal medicine patient care services, long-term care, sub-acute, and assisted living.
- Comply with Patient Care Protocols: Comply with patient care protocols for clinical practice at all sites of care.
- Enhance Cost-Effective Care: Comply with clinical outcomes targeted to facilitate changes in clinical practice that enhance cost-effective, high-quality care.
- Assist with Medication Monitoring: Assist the Section Chief with geriatric/internal medicine care issues and assist with medication monitoring in conjunction with the Pharmacy.
- Participate in Education Programs: Participate in the design and implementation of education programs for medical and other staff, as well as education programs for the community.
- Perform Other Duties: Perform other duties as assigned by the Chief, Division of Geriatrics.
- Degree from a Recognized Medical School: Possess a degree from a recognized medical school.
- Experience in Internal Medicine/Geriatrics: Experience in internal medicine/geriatric medicine and/or an affiliation with an academic institution.
- Effective Communication: Ability to relate effectively to all levels of the clinical and administrative structure of the medical center as well as other institutions affiliated with the Division of Geriatrics.
