About the Role

As a Certified Nursing Assistant (CNA) on our Hospice team, you will play a vital role in providing compassionate care to individuals with a life expectancy of six months or less. Our team specializes in end-of-life care and symptom management, serving patients in their homes, nursing facilities, and hospitals.

Responsibilities
  • Assess, plan, implement, coordinate, monitor, and evaluate patient care needs in collaboration with the interdisciplinary team.
  • Develop and maintain a patient-centered plan of care, utilizing the nursing process and relationship-centered care principles.
  • Provide direct patient care under the direction of the Unit Manager or Licensed Nurse, consistent with policies and procedures.
  • Document care provided as directed, in a manner consistent with guidelines.

Qualifications

To be successful in this role, you will need:

  • High School Diploma or equivalent.
  • Completion of State of Michigan approved CNA Training Course.
  • PROMETRIC Certified Nursing Assistant required within 90 days.
  • Driver's License within the State of Michigan.
Work Environment

This role is based at our Priority Health E Beltline location in Grand Rapids, with a full-time schedule of 40 hours per week, Monday through Friday, with on-call weekends.
