Responsibilities- Provide direct patient care under the supervision of a Registered Nurse (RN) or physician, including administering medications, performing vital signs, and documenting patient information.
- Collaborate with the healthcare team to develop and implement individualized patient care plans, ensuring that patients receive comprehensive and coordinated care.
- Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and implementing interventions to promote optimal outcomes.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions and address any concerns or questions.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care and safety.
- Graduate of an accredited practical nursing program.
- Current LPN licensure in the state of employment.
- Basic Life Support (BLS) certification through the American Heart Association.
- Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines.
- Excellent communication and interpersonal skills, with the ability to work collaboratively with patients, families, and healthcare team members.
