- Collaborate with patients, caregivers, and healthcare professionals to ensure seamless care coordination and communication.
- Prescribe medications and treatments as needed, while prioritizing patient safety and quality of care.
- Stay up-to-date with the latest medical research, technologies, and best practices to provide exceptional care and improve patient outcomes.
- Participate in ongoing quality improvement initiatives to enhance our services and meet the highest standards of care.
- Current NP license and RN license in good standing.
- Completion of a Nurse Practitioner accredited program.
- 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care.
- Knowledge of CPT, ICD-10, and HCPC codes, as well as clinical standards of care and NCQA requirements.
