Job Summary:

Aveanna Healthcare is seeking a skilled Occupational Therapist to join our team in providing high-quality home health care services to our patients. As an Occupational Therapist, you will play a vital role in helping our patients achieve their maximum potential and independence in their daily lives.

Key Responsibilities:

  • Conduct thorough assessments of patients' physical and cognitive abilities to identify areas of need and develop personalized treatment plans.
  • Deliver occupational therapy services to patients in their homes, including exercises, activities, and education to promote independence and functional ability.
  • Collaborate with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to ensure comprehensive care and coordination of services.
  • Document patient progress and treatment plans accurately and in a timely manner, ensuring compliance with regulatory requirements and company policies.
  • Participate in discharge planning and education of patients and their caregivers on home exercise programs and other necessary interventions.
  • Provide consultation and guidance to other healthcare professionals, including occupational therapy assistants and home health aides, as needed.

Requirements:

  • Master's or Doctoral degree in Occupational Therapy from an accredited program.
  • Current, unrestricted state license as an Occupational Therapist.
  • Valid CPR certification.
  • Strong communication and interpersonal skills, with ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with strong organizational and time management skills.

Preferred Qualifications:

  • Experience in home health care and completion of OASIS documentation.
  • Strong knowledge of occupational therapy principles and practices, with ability to apply evidence-based interventions.
