Your expertise will help patients regain mobility and independence in the comfort of their own home.

Key Responsibilities:

* Conduct comprehensive assessments of patients' physical abilities, mobility, strength, balance, and functional limitations to develop personalized treatment plans.
* Design and implement therapeutic interventions, including exercises, gait training, and mobility techniques, to improve patient strength, coordination, and range of motion.
* Educate patients and their families on home exercise programs, techniques for improving mobility, and safety measures to prevent falls or injuries in the home.
* Collaborate with physicians, nurses, and other members of the healthcare team to ensure a coordinated and comprehensive approach to patient care.
* Regularly assess and monitor patient progress, adjusting treatment plans as needed to ensure optimal recovery and achievement of therapy goals.
* Document patient evaluations, treatment plans, and progress notes in the electronic health record (EHR), ensuring compliance with home health regulations and standards.

Requirements:

* Strong understanding of physical therapy principles, rehabilitation techniques, and home health care protocols.
* Excellent communication and interpersonal skills for working with patients, families, and healthcare teams in a home health environment.
* Proficiency in conducting assessments, creating individualized therapy plans, and implementing physical therapy interventions.
* Active [STATE] Physical Therapist (PT) License in good standing.
* A minimum of 2 years of experience as a Physical Therapist (PT), preferably in a home health or rehabilitation setting.
* Completion of an accredited physical therapy program (Master's or Doctoral degree).
* Certification in Basic Life Support (BLS).
* Experience with home health documentation systems (e.g., EPIC, Homecare Homebase, etc.).
