Job Summary:

We are looking for a registered nurse with experience in skilled nursing to provide high-quality care to our patients in their homes. The ideal candidate will have strong clinical skills, excellent communication skills, and the ability to work independently.

Responsibilities:
  • Conduct initial and ongoing supervisory field visits to patients' homes to provide assessment, training support, and in-home coordination.
  • Collaborate with the primary care physician and clinical team to perform a full patient assessment and develop a care plan.
  • Identify continued training needs of the home health aide and document the information electronically.
  • Deliver the start of care packet and provide an overall review with the patient/caregiver on initial visit.
  • Participate in quality assurance and education programs as requested by management.
Requirements:
  • License and current registration to practice as a Registered Professional Nurse in the State of New York required.
  • Associate's Degree in Nursing from an approved program required.
  • Home care experience in a licensed home care service agency or CHHA settings preferred.
