Key Responsibilities
  • Conduct thorough psychosocial assessments of new patients to develop effective treatment plans.
  • Maintain open communication with patients' families and significant others to provide education, support, and advocacy.
  • Collaborate with community-based social and mental health agencies to ensure seamless care transitions.
  • Facilitate discharge planning and conduct post-discharge follow-ups to reduce re-hospitalization rates.
  • Lead daily groups with patients to promote socialization and skill-building.
Requirements
  • Master's degree in Social Work (MSW) from an accredited institution.
  • Unrestricted licensure in social work (LMSW or LPC) or equivalent.
  • At least one year of experience in a hospital setting with psychiatric experience preferred.
  • Successful completion of CPR and Crisis Prevention Intervention Training within 30 days of employment.
  • Availability to work a full-time schedule with some weekend hours.
