Position Overview:

The primary responsibility of this role is to support clients residing in the community by connecting them to essential medical, behavioral health, housing, social, employment, educational, and other services necessary for fulfilling basic life requirements.

Sign-on Bonus: $3000 distributed over the first year of satisfactory employment.

Qualifications:

  • A BA/BS in a human services discipline with a minimum of one year of experience in a behavioral health environment is required, or an AA/AS in a human services field with a minimum of two years of experience in a behavioral health setting.
  • Must possess a solid understanding of the behavioral characteristics and psychosocial needs of the target demographic.
  • Excellent written and verbal communication skills are essential for maintaining documentation and collaborating effectively with clients, providers, and the healthcare community.
  • Knowledge and skills in behavioral health are necessary.
  • Bilingual proficiency in English and Spanish is advantageous.
  • Must have access to a personal vehicle, a valid driver's license, and a driving record that meets our insurance standards.
  • A Criminal History Background Check and fingerprinting are mandatory.
