Hospice and Palliative Medicine Nurse Practitioner

We are seeking a skilled and compassionate Hospice and Palliative Medicine Nurse Practitioner to join our team at MultiCare Health System.
About the RoleThis is a challenging and rewarding opportunity to work with patients and families in need of end-of-life care. As a Hospice and Palliative Medicine Nurse Practitioner, you will provide high-quality, patient-centered care to individuals with serious illnesses, focusing on symptom management, pain control, and quality of life.
Responsibilities- Assess and manage patients' physical, emotional, and spiritual needs
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Provide education and support to patients and families on end-of-life care and advance care planning
- Collaborate with healthcare providers to ensure seamless transitions of care
- Participate in quality improvement initiatives to enhance patient outcomes and experiences
To be successful in this role, you will need:
- A Master's degree in Nursing or a related field
- Current licensure as a Nurse Practitioner in the state of Washington
- Experience in hospice and palliative care, with a focus on symptom management and pain control
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
