We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Skyline Nursing and Rehabilitation Center. As an LPN, you will play a vital role in providing exceptional nursing care to our residents, collaborating with our healthcare team to develop individualized treatment plans, and supervising and evaluating Certified Nursing Assistants (CNAs) to ensure high-quality care.
Responsibilities- Provide high-quality nursing care to residents, including administering medications, wound care, and vital sign monitoring.
- Collaborate with the healthcare team to develop and implement individualized treatment plans for each resident.
- Supervise and evaluate CNAs to ensure they are delivering care that meets our high standards.
- Oversee fall-risk and pressure ulcer prevention programs, actively working to prevent and manage these conditions.
- Conduct thorough evaluations of each resident's medical status upon admission and throughout their course of treatment, documenting any changes and adjusting care plans as necessary.
- Active license to practice as an LPN.
- Experience in long-term care, skilled nursing, or rehabilitation facilities is preferred.
