Job Title: Clinical Care Specialist

Job Summary:
We are seeking a skilled Clinical Care Specialist to join our team at The Rehab Center at Bristol. As a Clinical Care Specialist, you will be responsible for providing high-quality patient care and support to our residents.

Responsibilities:

* Administer medications in an accurate and timely manner to residents in accordance with physician's orders.
* Provide emergency care to residents when necessary.
* Requisition adequate medications and supplies to administer medications in a timely manner.
* Supervise other employees and caring for residents.
* Monitor and document effectiveness of treatment and medication.
* Maintain a courteous, and professional appearance at all times.
* Follow facility policy and procedures as outlined in Employee Handbook.

Requirements:

* High School Diploma or GED equivalent
* Current RN License/CPR Certification
* Professional, friendly, dependable and punctual individuals with a current RN license.
* Candidates for this position will possess knowledge of computer documentation as well as writing documentation skills.
* Administer medication and treatments to resident as order by a physician or qualified practitioner.
* Supervise CNA in caring for residents.
* Complete and document comprehensive assessments of residents needs as related to medication administration.
* Follow a comprehensive plan of care for individual residents
