Responsibilities:
  • Plan of Care: Develop and implement individualized speech therapy plans to improve patient outcomes.
  • Patient Outcomes: Use evidence-based practices to improve swallowing, breathing, cognition, speech, and communication functions.
  • Minimize Patient Risk: Educate patients and families on preventative measures to reduce re-hospitalizations.
  • Family Educator/Advocate: Counsel patients and families on disease management and provide support throughout the care process.
  • Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations.
Requirements:
  • Current license as a Speech Therapist in the state of employment.
  • Minimum one-year experience as a Speech Therapist in an acute care setting, with home care experience preferred.
  • Valid driver's license and auto insurance in your name as a driver.
  • Capable of all physical demands of the job.
