Duties and Responsibilities:
- Patient Assessments: Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing targeted care plans.
- Medication Management: Administer medications and treatments as ordered by physicians, monitoring patient responses and adjusting plans as necessary.
- Care Coordination: Collaborate with patients, caregivers, and healthcare providers to ensure seamless care transitions and optimize patient outcomes.
Education and Experience Requirements:
- Graduation from an accredited school of nursing and Illinois licensure as a Registered Nurse or Licensed Practical Nurse.
- One year of experience in home care or hospital settings (preferred).
