About the Role

We are seeking a dedicated and compassionate Nurse-Leader to join our team at Hospice of the Chesapeake. As a Clinical Manager, you will be responsible for leading and guiding staff in the delivery of high-quality patient care.

Key Responsibilities
  • Lead and manage a team of healthcare professionals to ensure the delivery of safe, effective, and appropriate care to patients.
  • Develop and implement care plans that meet the unique needs of each patient.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Conduct patient visits to offer support and expertise to nursing staff, patients, and families.
  • Evaluate the quality of patient care through direct staff observation and review of nursing documentation.
Requirements
  • Bachelor's degree in nursing, healthcare administration, or behavioral health.
  • 3-5 years of nursing experience in a nursing home, assisted living center, or other post-acute, long-term care facility.
  • 2-3 years of supervisory experience.
  • Excellent oral, written, and interpersonal communication skills.
  • Knowledge of Medicare, Medicaid, private insurance, and other community programs and resources.
