Job Title: Physical Therapy Assistant

We are seeking a skilled Physical Therapy Assistant to join our team at Cynet Health. As a Physical Therapy Assistant, you will play a vital role in providing high-quality patient care and supporting our rehabilitation team.

Job Summary:

The Physical Therapy Assistant will work closely with our physical therapists to develop and implement individualized treatment plans for patients. This will involve conducting assessments, providing therapy sessions, and documenting patient progress.

Responsibilities:
  • Conduct comprehensive assessments of patients to identify their physical therapy needs
  • Develop and implement individualized treatment plans in collaboration with physical therapists
  • Provide physical therapy sessions to patients, including exercises, stretches, and other interventions
  • Document patient progress and maintain accurate records
  • Communicate effectively with patients, families, and healthcare team members
  • Participate in interdisciplinary team meetings to discuss patient care and treatment plans
Requirements:
  • State license verification required
  • Proof of a recent negative COVID-19 test (72 hours before start date), 1 Step PPD or Quantiferon, or proof of positive PPD and Chest X-Ray
  • Completed and attached prescreen form
  • Resume
  • Three references
Preferred Qualifications:
  • Skilled Nursing Facility (SNF) experience preferred
