We are seeking a skilled Nurse Practitioner to join our team in providing high-quality, patient-centered care in the comfort of patients' homes. As a member of our HouseCalls product, you will conduct annual clinical assessments, identify health risks, and coordinate follow-up care to improve patient outcomes.
Key Responsibilities:- Conduct in-home assessments, including medical history review, medication reconciliation, and comprehensive physical exams
- Perform evidence-based practice screenings and point-of-care testing as needed
- Develop and implement care plans to address patient needs and improve health outcomes
- Communicate with patients' primary care physicians to ensure seamless care coordination
- Identify and address urgent and emergent situations, providing timely interventions as needed
- Active NP license and national certification in Family, Geriatrics, or Adult-Gerontology Primary Care
- Ability to work autonomously and make clinical decisions with minimal supervision
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
- Access to reliable transportation and ability to navigate various dwelling conditions
- Experience in gerontology, cardiology, internal medicine, or endocrinology
- Home health care or home visit experience
- Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages
