American Medical Staffing is seeking a skilled and compassionate Registered Nurse to join our team as a Hospice Nurse. As a Hospice Nurse, you will provide high-quality, patient-centered care to individuals in their homes, promoting independence and optimal health outcomes.
Responsibilities:
- Conduct comprehensive assessments of patients' health status and develop individualized care plans in collaboration with the interdisciplinary team.
- Provide skilled nursing care to patients in their homes, including medication management, wound care, and infusion therapy.
- Perform OASIS assessments and documentation accurately and timely to ensure compliance with regulatory requirements.
- Educate patients and their families on disease management, medication administration, and other aspects of health to promote self-care and independence.
- Monitor patients' progress and response to treatment, adjusting care plans as needed to achieve desired outcomes.
- Provide emotional support and counseling to patients and families coping with chronic illness or end-of-life care.
Requirements:
- At least 1 year of experience in a Medicare-certified home health agency.
- Comfortable with OASIS (Outcome and Assessment Information Set).
- Valid RN license and willingness to obtain MA licensure.
