We are seeking a skilled Nurse Practitioner to join our HouseCalls team at UnitedHealth Group. As a key member of our team, you will conduct in-home assessments, provide evidence-based practice screenings, and communicate findings to inform members' PCPs of potential gaps in care.
Key Responsibilities- Conduct comprehensive in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and physical exams.
- Perform evidence-based practice screenings, including point-of-care testing as appropriate.
- Identify diagnoses to inform care management and active medical management.
- Communicate findings to inform members' PCPs of potential gaps in care.
- Intervene in urgent and emergent situations as needed.
- Educate members on disease processes, medication, and compliance.
- Active NP license or ability to obtain by start date.
- Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or ability to obtain national certification and/or NP license in state of assignment by start date.
- Ability to complete physical requirements of the job, including lifting a 30-pound bag, navigating stairs, and sitting, standing, and kneeling for physical assessments.
- Reliable transportation for home visit assessments.
- May be requested to obtain additional licensure in other geographic areas.
- 1+ years of clinical experience in highest level of education, clinical setting.
- Experience in gerontology, cardiology, internal medicine, or endocrinology.
- Home health care or home visit experience.
- Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages.
- Ability to travel throughout the state, beyond assigned region, up to 5% of the time, with advanced notice and potential for additional compensation incentives.
UnitedHealth Group offers a comprehensive benefits package, including a competitive salary range of $88,000 to $173,200 annually, based on experience and qualifications. Additionally, we offer incentive and recognition programs, equity stock purchase, and 401(k) contribution, subject to eligibility requirements.
