About the Role:- Provide comprehensive nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of care.
- Develop and implement individualized care plans that address the unique needs and goals of each patient.
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure seamless care coordination.
- Conduct regular home visits to assess patient progress, provide education, and make adjustments to care plans as needed.
- Document patient information accurately and thoroughly, maintaining confidentiality and adhering to regulatory requirements.
- Diploma, Associate, or Bachelor's degree in Nursing
- Minimum of one year of nursing experience, preferably in home health or a related field
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Strong organizational and communication skills
- Valid driver's license and reliable transportation
- A competitive salary range of $69,800 - $96,200 per year
